This service helps you with those difficult discharges. When it comes to moving a patient to a different provider- specifically LTC, AL, GH, or HH. Third party involvement is more credible than doing it yourself. Get your time back and allow GI to do the hard work for you. 

What you will get:

An associate to assist you in your LTC placements. Some families are reluctant to work with discharge planners due to time crunch and not knowing. At Globally, we meet families at their chosen facility already discussed with you and your team, or at the hospital, or virtually. We go over ratings, their rights, cost, Medicare/ Medicaid/private pay patient education, answering all questions pertaining to LTC, and preplacement tours to help ease their transition.
